Sneha is a bold character and she does the extreme, knowing that it intimidates Suresh. At few instances, she has asked Suresh whether she could continue sex with gowtham or not.. few instances.. she`s into cuckoldry, enjoying her freedom, convincing herself that Suresh is Manageable.. 

Truly to say, I have witnessed this Sneha kinda attitude/character and heard many as such. 
If Sneha is to choose between Suresh or Gowtham, she would flatly opt for Gowtham. 

Considering practical approach, Suresh is a dutiful husband, who wants to make Sneha happy (curing his erectile issues, so that Sneha could enjoy im longer) and knowing Sneha abrupt affinity towards Gowtham, what more could he could do...  Suresh could ignore/leave Sneha, which would leave Ramya in trauma by this, but, Suresh has to plan this carefully. Although, his love to Sneha still hanging, but cautious enough to rupture at the right time.

Inspite of repeated reminders to Sneha, she still despises him. 

In real life, many such incidents of cuckoldry had taken toll in their life, understanding is important. Here, Sneha is yet to understand Suresh.
