Regarding Meera's perceived “mistakes”, I'm not suggesting she necessarily sees them as such. She and Krish played a game, using others for their pleasure. As long as they were honest with each other, there wasn’t an issue. But when they started hiding their true feelings, insecurities, and suspicions, problems arose. Meera is now wondering where she went wrong, where she should have applied the brakes to avoid this situation. She's reflecting on what compounded her misery, where she should have recognized the issues, and what she could have avoided doing or saying to prevent Krish from feeling this way.

In short, I'm attempting to retroactively adjust my understanding of the story to fit the given ending, focusing on the unresolved questions from the final chapters.
There is a concept and book by that name - The Tipping Point. May be Alpine Heights proved to be the tipping point for Meera, and anniversary for Desai. Krish still struggled with his inner demons.
