Very nice story. Had been waiting for the updates for a few months. Now, please give updates regularly.
Here are a few suggestions.
(i) Make her completely discard the clothes and remain continuously naked in her hospital ward. She should feel no shame about being naked in front of doctors and hospital staff.
(ii) As she is enjoying the dog food, Sam should tell her that the food she has been eating for a while is actually dog food. But she shouldn't care and continues to eat that food. Moreover, now serve the food to her in a dedicated dog bowl. Even nurses should throw dog biscuits in front of her, and she will eat them directly from the floor without caring what she is doing.
(iii) Make her juniors visit the hospital whom she used to abuse and mistreat, like her driver and peon etc. When they see her completely naked in a dog-like situation, they will feel great relief and enjoy it. Create a situation where she licks their boots as well.
(iv) Would be a little extreme, but if you want to consider it, then take her in a public place in this situation, like in the hospital garden, etc. As she is constantly getting doses of dog medicine, she should feel sexually attracted to the dogs. So get her fucked by the local dog there.
And later put the blame for her whole condition on the infection she got due to a dog bite.
